Your primary physician has elected to take part in this new Medicare program. Your physician believes you will benefit from the care coordination and preventive services offered by ABC Network.

No. This is a voluntary alignment program. You may continue to see your primary clinician and not participate in the program.

No. You can continue using all of the same providers you have used in the past. However, your primary physician may recommend a provider for you. The choice is always yours.

Not at all. You still have Traditional Medicare and if you have any supplemental or prescription coverage, nothing will change with those plans either.
