Arizona Best Care Network, LLC (ABC Network) is a value-based accountable care organization that supports health care providers to provide the best care possible to their patients. We support person-centered care for individuals, better health for populations, and lowering health care costs by supporting early intervention and comprehensive care management. We provide analysis and insight to improve the quality of care and the health and wellness of our Medicare members.
Adding Value to Patient Care
The ABC Network was created in late 2020 and began its collective work on April 1, 2021. The ABC Network combines the expertise of numerous health care professionals and community based health centers along with our data scientists, data analytics, and performance improvement specialists, intending to create healthier outcomes for our patients while reducing their burden of care and costs. ABC Network, LLC is an Arizona Limited Liability Company.
What is a Value-Based Care Network?
A Value-Based Care Network is a health care delivery model where providers (doctors, clinics, hospitals, nurses, etc.) come together with one goal: Provide high-quality, efficient, coordinated care to Medicare beneficiaries with a proactive team-oriented and data-driven approach to keeping people healthy.
